The Swing Bed Program is designed for patients who no longer require acute hospital care but still need skilled care before returning home.

Our dedicated team is here to ensure your smooth transition home. Even if you’ve received inpatient care or surgery at another hospital, you can choose Eaton Rapids Medical Center for your sub-acute rehabilitation needs.

Stays are typically short-term and can be used for a variety of conditions including, but not limited to:

  • Stroke
  • Heart disease
  • Joint replacement & other surgeries
  • Respiratory disease
  • Newly diagnosed diabetes
  • Fall injuries

A variety of insurance plans are accepted, including Medicare, Medicare Plus Blue, Humana, and many commercial insurance products.

What to Expect

Average Length of Stay:

1-2 Weeks

Physical and Occupational Therapy (PT/OT):

Extensive therapy provided for up to one hour per day

Nursing Assistants (RN/CNA):

Available 24/7 to assist with care

Provider Visits:

Weekly visits to assess patient progress

Social Worker:

Available five days a week to manage discharge planning and address concerns

Care Conferences:

Held weekly with the patient, family, and care team to discuss goals and upcoming plans

Please Note: We recommend that patients bring 5-6 changes of clothing, comfortable walking shoes, nightwear, and undergarments. Bedding and linens will be provided.

On-site Services

Transitioning Home

When our goals are met, we assist in making the transition home easier for both patients and their families. As patients prepare to leave, support may include: skilled home care, durable medical equipment, follow-up appointments, and coordination of community services.
